• Assist with the implementation and facilitation of services to members in the program
• Collaborate with members/families, physicians, and the healthcare team to coordinate care
• Advocate for members/families to ensure resource utilization and evaluation of services
• Document care accurately and comprehensively based on standards of practice
• Provide field-based and/or telephonic case management activities specific to the program

• Bachelor’s degree in a health-related or behavioral science field
• Minimum of three (3) to four (4) years of broad clinical experience
• Minimum of three (3) years of experience in home care, discharge planning, or case management
• Minimum of two (2) years of experience in the managed care industry
• Active Unrestricted NJ SW License preferred; restricted licenses will not be considered

• Experience with the elderly frail population
• Strong knowledge of managed care principles and concepts
• Good presentation skills and the ability to work with limited daily supervision
