This vital role involves utilizing community outreach to provide a range of healthcare education and services to individuals in need, with particular emphasis on wound management. Other key work will include linking clients to substance use disorder treatment, support for infectious disease management, and other public health assistance.
Key ResponsibilitiesCare and Treatment:
Helps ensure clients get the health care they need. Conduct in person assessments and when necessary, provide direct care to clients, including managing and navigating care for acute and chronic illnesses. Help clients manage medical interventions, and advocate for the use and training of naloxone. Provide direct wound management assistance in a community setting via an agency mobile vehicle, care includes changing wound dressings, observing wounds for signs of infection and educating clients on best practices to promote healing and wellness.
Patient Engagement:
Using evidence-based practices, educate patients on wound prevention, infection control, harm reduction strategies, and medication adherence.
Public Health Assistance:
Assist in the prevention, diagnosis, and management of infectious diseases among clients. Educate on safe sex practices and safe needle use. Collaborate with other healthcare professionals and community organizations to deliver integrated care and public safety support.

Education:
Work with clients to educate on health promotion, disease prevention, and harm reduction strategies.
Community Outreach:
As a member of the Mobile Unit team, the LPN will provide essential wound care, health assessments, and harm reduction services as part of a mobile community health unit serving individuals experiencing homelessness, substance use disorders, and other vulnerable populations within Trenton and the surrounding Mercer County area.
Documentation:
Document patient care and maintain accurate records in compliance with organizational and regulatory requirements.
Training:
Provide training to staff on essential health and safety topics including but not limited to CPR, infectious disease control, workplace safety etc.
- Current and valid LPN license in the state of New Jersey.
- Experience in harm reduction preferred, strong aspiration to join harm reduction field required.
- Experience working with people experiencing homelessness, substance use disorders, or public health challenges.
- Clinical skills and knowledge of primary care, infectious diseases, CPR, and substance use treatment.
- Excellent communication and interpersonal skills to engage with a diverse client population.
- Commitment to serving vulnerable populations with dignity, respect, and cultural sensitivity.
- Work with a team of RNs, OTs, PTs, SLP's and HHAs to deliver quality patient care
- Take patient vitals, do assessments, document progress and report changes to RN supervisor
- Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
