Infusion RN; Registered Nurse

The Home Infusion RN is a member of the YNHH Home Infusion Therapy team. Responsible for the delivery of high-quality care as it relates to delivery of infusion therapy in the home or alternate setting. Provides education to patient and/or caregiver on proper handling, storage and administration of your infusion medications as well as signs and symptoms of access device complications.

Coordinates patient care and maintains communication with the physician, pharmacy team as well as nursing agency(s).

• Provides home infusion nursing (and alternative setting) intravenous, subcutaneous, oral, etc. medication administration for YNHHS Home Infusion Pharmacy patients.
• Teaches patients and/or family/caregivers about the patient's care needs (including self-administration when applicable) and disease prevention.
• Follows and adheres to infection control measures and standards for home care nursing and IV nursing services.
• Maintains accurate and timely clinical nursing records according to YNHHS Home Infusion Pharmacy policies.
• Adheres to ethical nursing standards and maintains confidentiality of patients and personal information at all times.
• Performs other duties as directed by supervisor and represents the home infusion pharmacy in a professional and positive manner.
• Stays abreast of developments in the field of infusion therapy and complies with all YNHHS policies and directives for the fulfillment of the YNHHS objectives.
• Communicates any problems incurred directly to the Manager of Home Infusion Therapy.

• Nursing diploma or associate degree in nursing. Bachelor's degree in nursing preferred. CRNI certification a plus.

• Three years of IV related experience. Previous experience in the home care or home infusion setting is desirable. Case Management experience a plus.

• Registered Nurse with valid (or eligible) Connecticut licensure required.

• Experienced in the care of peripheral and central lines. The ability to troubleshoot potential problems that may arise in dealing with IV therapy. Knowledge of medications, indications, dosage ranges, side effects, and potential toxicities. Ability to work independently. Must have excellent customer service skills (both verbal and written) and effective interpersonal skills. Proficient computer skills.

• Must be capable of lifting 20 lbs, standing, walking, and bending.
