Registered Nurse After Hours Visit/On Call

• Assess the assigned case load of home‑health care patients to identify physical, psychosocial, and environmental needs.
• Make initial evaluation and re‑evaluate patients’ nursing needs during each visit.
• Communicate significant findings, problems, and changes to the clinical supervisor, physician, and other personnel.
• Counsel, instruct, and implement the plan for patient safety, utilizing patient information, family, and community resources.
• Participate in implementation of the plan of care to ensure quality, continuity, and achievement of outcomes.
• Identify deviations from normal condition and initiate appropriate actions.
• Verify the plan of care before each visit and provide care according to physician orders, assessment data, and established guidelines.
• Perform other duties or projects as assigned.

• Graduate of an accredited certificate, diploma, associate’s, or baccalaureate nursing program.
• Minimum 1 year of experience as a nurse in a clinical care setting.
• Home‑health experience preferred but not required.
• State‑licensed Registered Nurse.
• Ability to travel approximately 50% of the time.
• Excellent verbal and written communication skills.

You will report to the Branch Manager or Clinical Manager.
