Are you a compassionate and driven Physical Therapist looking to make a meaningful impact in the lives of older adults? Join our dedicated rehabilitation team in a welcoming and supportive Skilled Nursing Facility (SNF) where your expertise is valued, and patient care is the top priority.

We are seeking a licensed Physical Therapist (PT) to provide comprehensive rehabilitation services to residents with a variety of medical conditions. The PT will work closely with an interdisciplinary team to develop personalized care plans that help residents regain strength, mobility, and independence.

• Evaluate residents and develop individualized treatment plans
• Deliver evidence-based physical therapy services
• Document progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, and other rehab team members
• Educate residents and families on therapy goals and home exercises

• Current and valid Physical Therapist (PT) license in CT
• Graduate of an accredited Physical Therapy program
• Previous experience in SNF or geriatric setting preferred (new grads welcome!)
• Strong communication and interpersonal skills
