Primary Care RN Team Lead (Full Time)
Position Summary
The Registered Nurse (RN) Team Lead plays a crucial role in overseeing and guiding a team of nurses to deliver exceptional patient care. This position combines clinical expertise with leadership skills to ensure that both patient care standards and operational goals are met. The RN Team Lead will collaborate with healthcare professionals, manage resources, and act as a mentor to nursing staff, promoting a culture of safety, respect, and continuous improvement.
The role is located at the primary care office in Groveton, NH, and includes triage phone calls, patient communication, referral processing, lab draws, nurse visits, and occasional provider rooming.
- Associate of Science in Nursing (ASN) from an accredited institution.
- Bachelor or Master’s degree in Nursing or Healthcare Management preferred.
- Certification in leadership or nursing specialty preferred (e.g., Nurse Executive, CNL).
- Current Registered Nurse (RN) license in the state of practice.
- Basic Life Support (BLS) or equivalent CPR/AED certification, renewed regularly.
- Strong leadership and team management capabilities.
- Excellent communication and interpersonal skills.
- Ability to prioritize and manage multiple tasks efficiently.
- Proficiency in electronic medical records (EMR) systems.
- Strong problem-solving and critical-thinking abilities.
- Minimum of 5 years of clinical nursing experience in an acute care setting.
- Minimum of 2 years in a supervisory or leadership role.
- Lead and supervise nursing staff, ensuring high standards of patient care.
- Coordinate daily nursing operations, including patient assignments and staff scheduling.
- Serve as a clinical resource and mentor, assisting staff with care plans.
- Conduct performance evaluations, provide feedback, and implement development plans.
- Ensure compliance with regulatory requirements and quality standards.
- Collaborate with interdisciplinary teams to address complex patient needs.
- Participate in recruitment, selection, and onboarding of nursing staff.
- Develop and implement patient care programs and initiatives.
- Facilitate effective communication among staff, patients, and families.
- Monitor and improve patient outcomes through data analysis and quality improvement.
- Manage resource allocation efficiently.
- Address and resolve clinical and operational issues promptly.
- Lead unit meetings and foster a team-oriented environment.
- Participate in budget planning and management.
- Advocate for continuing education and professional development of staff.
All new hires are required to attend a mandatory three‑day orientation at Weeks Medical Center in Lancaster, NH. Attendance on all three days is required for onboarding completion. If Weeks Medical Center is not your designated home base, you will be eligible for travel reimbursement per the standard travel policy.
