Overview

Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and offers work‑life balance flexibility (no on‑call or weekend shifts). We are seeking a part‑time to full‑time (24–40 hours per week) provider for our Lafayette Center located in Franconia, NH. This position can be filled by a nurse practitioner or physician assistant. New graduates are welcome to apply.

The hourly range for this position is $56–$58 per hour.

Responsibilities
  • Directly report to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
  • Collaborate on‑site with the Center Leadership Team for center‑specific goals and priorities.
  • Work with the Medical Director and/or Attending Physician for patient‑specific medical care.
  • Provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
  • Work collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving the lives we touch through high‑quality health care and everyday compassion.
Qualifications
  • Graduate from an accredited School of Nursing program and be a licensed registered nurse.
  • Certified as a Nurse Practitioner with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.
  • Or, a graduate from an ARC‑PA accredited program who has passed the PA National Certifying Exam (PANCE) and holds state licensure. NCCPA certification is required.
  • Minimum two years of experience working with adult or geriatric populations is preferred.
  • In states where practice allows, an APRN must have an unrestricted professional license and a DEA registration for each state in which they practice. PAs must likewise hold an unrestricted professional license and DEA registration for each state in which they practice.

Posted Salary Range: USD $56.00–$58.00 per hour.
