Please Note: Completing a Volunteer Application does not ensure placement. The Human Resources Department reserves the right to deny placement if there is reason to believe the Volunteer Applicant is not an appropriate match for MSB, as well, the Volunteer Applicant is not obligated to accept any position that is offered. All applications are held for the duration of the school year and are referred to when applicable volunteer opportunities become available.

As a Volunteer Applicant,

I hereby certify that the answers on the application and any answers resulting from interviews are true and correct and that any misrepresentations or omissions of facts, misleading, or false information on my part will be grounds for dismissal as a volunteer. Acceptance as a volunteer is contingent upon satisfactory references, verification of information submitted on the application and satisfactory completion of mandatory requirements.

I hereby give permission to The Maryland School for the Blind to perform a check of my background, including my criminal record, driving record, current employment and volunteer history, professional status, and to contact my personal references, and other persons or sources as appropriate for the volunteer position(s) in which I have expressed an interest.

I understand that I am not obligated to agree to this background check, but that refusal to do so will exclude me from consideration for volunteer service.

I understand that information collected during this background check will be limited to that appropriate to determining my suitability for particular types of volunteer service and that all such information collected during the check will be kept confidential.

I hereby also extend my permission to those individuals or organizations contacted for the purpose of this background check to give their full and honest evaluation of my suitability as a volunteer and any such other information as they deem appropriate. I release those thus contacted from any and all liability that might arise as a direct or indirect result of responding to an inquiry made by MSB or its Volunteer Department.

I understand and accept that the responses from the individuals or organizations thus contacted are kept confidential and not shared with me. I hereby declare and affirm under the penalties of perjury that the information I have provided in this application is true, correct and complete.

The criteria to volunteer are:

• Minimum age of 16 years old. If volunteer is a high school student, a letter of recommendation from a teacher or guidance counselor must accompany the application.
• Minimum commitment of 4 hours per week for 3 months
• Successful completion of the application, interview, and screening process, including fingerprinting for background check, pre-placement drug screen/physical/TB test, and a CPS background check.
