We are grateful for the many volunteers who offer invaluable support to our patients and families across Hospice, Administration, and Home Health Care. As a Hospice volunteer you undergo specialized training to address the unique challenges of end-of-life care as part of the Hospice team. Your primary role is to offer companionship to both patients and their families, delivering non-medical support during this sensitive time.
This role includes visiting patients and providing respite to family members and caregivers, and may also involve assistance within the administrative sphere. This is strictly a volunteer, unpaid position.
- Follows through with commitments made with clients and families. Visits as scheduled and provides support such as errands, light household tasks, meal preparation, respite, companionship, and caregiver support.
- Provides continued support in inpatient settings when the patient’s needs require it.
- Maintains strict confidentiality of all information, including client and family names, and medical and personal information. Adheres to HIPAA requirements of the Agency.
- Identifies physical, spiritual, and social services needs and communicates with the Volunteer Coordinator.
- Works with the IDG team (Interdisciplinary Group) to update the individualized care plan.
- Participates in Interdisciplinary Group meetings as requested.
- Ensures that all required documentation is completed and submitted on time.
- Communicates with the Hospice Volunteer Coordinator regularly for support and supervision.
- Education/Certification:
Satisfactory completion of the prescribed Volunteer Training Program. - At the discretion of the Hospice Director, training may be waived for volunteers who have documented completion of Volunteer Training at another hospice program.
- Possess a valid unencumbered driver’s license in the State of NH.
- Access to an operable, insured motor vehicle. Must provide documentation of auto insurance coverage.
- Life experience and a willingness to be open and see things in a new and unique way.
- Excellent observation, verbal and written communication skills, problem solving, and time management skills.
- Prolonged or considerable walking or standing is possible.
- Able to lift supplies; at times may be required.
- Considerable reaching, stooping, bending, kneeling, or crouching may be possible.
- Visual acuity and hearing to perform required skills.
- If volunteers support the bereavement program, the coordinator is responsible for supervision and tracking hours and performance.
