Job Summary

RN - LTC / SNF - Registered Nurse

Shift: Days

Hours:

7:5a - 7:30p

Pay: $38 hourly

Contract:

21 weeks

Responsibilities

• Provide direct patient care in long-term care and skilled nursing facilities.
• Achieve performance targets and contribute to team success.
• Administer medications and monitor patient responses to treatments.
• Engage in continuous learning and professional development activities.
• Collaborate with healthcare team to develop and implement care plans.
• Educate patients and families on health management and wellness.
• Document patient care activities and maintain accurate health records.
• Participate in quality improvement initiatives to enhance patient care.
• Utilize data to assess patient outcomes and improve care strategies.
• Monitor vital signs and report changes in patient conditions.
• Assist in training and mentoring new nursing staff.
• Ensure compliance with healthcare regulations and standards.

Experience/Qualifications

• Current RN license and certification for long-term care nursing.
• Strong commitment to continuous learning and professional development.
• Proven track record of achieving performance targets in nursing roles.
• Proven experience in skilled nursing facilities or long-term care settings.
• Excellent communication skills for patient and family education.
• Ability to monitor vital signs and report changes promptly.
• Ability to collaborate effectively with a multidisciplinary healthcare team.
• Familiarity with healthcare regulations and compliance standards.
• Experience in documenting patient care and maintaining health records.
• Experience in quality improvement initiatives and patient care enhancement.
