An exciting opportunity has arisen for a Band 6 Physiotherapist to join the Discharge Planning Team in Epsom
.
This ongoing, full-time locum role offers a competitive hourly rate of £29.74 and the chance to work in a fast-paced acute environment where you will play a key role in supporting safe, timely, and effective hospital discharges
.
You will work across medical wards, helping patients regain independence and ensuring smooth transitions from hospital to home or onward care.
Responsibilities- Manage discharge planning for a varied caseload of patients, ensuring safe and timely discharges.
- Provide specialist physiotherapy assessment and treatment to patients across medical wards.
- Work collaboratively within a multidisciplinary team to develop and implement effective discharge and rehabilitation plans.
- Complete functional assessments and implement treatment programmes to maximise patient independence.
- Maintain accurate and timely clinical documentation in line with professional and organisational standards.
- Contribute to service efficiency and patient flow across the hospital.
- HCPC registration
. - Full UK driving licence (required to travel between locations).
- Previous experience working at Band 6 level in an acute or discharge-focused setting is desirable.
