We are seeking a dedicated Occupational Therapist to join our team. In this role, you will provide high-quality therapy services to help patients regain independence and improve their quality of life. You will evaluate patient needs, develop treatment plans, and collaborate with healthcare professionals to ensure comprehensive care.
Key Responsibilities- Evaluate patients’ functional status and home environment for hazards or barriers to independence.
- Develop individualized treatment programs and establish goals for improved function.
- Teach new skills or retrain patients in daily activities lost due to illness or injury.
- Fabricate splints and instruct patients in the use of adaptive equipment, prosthetics, and orthotics.
- Maintain accurate clinical records and communicate changes in patient condition to physicians.
- Participate in care planning, discharge planning, and interdisciplinary meetings.
- Supervise Certified Occupational Therapy Assistants and home health aides as required.
- Ensure compliance with organizational policies and state regulations.
- Graduate of an accredited Occupational Therapy program.
- Licensed with the NH State Board.
- Minimum of 2 years of clinical experience as an Occupational Therapist.
- Current CPR certification.
- Valid driver’s license and reliable, insured vehicle.
- Strong verbal, written, and organizational skills.
This is a traveling position; reliable transportation is required for varied weather conditions.
Physical Requirements- Ability to walk or stand for prolonged periods.
- Lift, position, or transfer patients up to 50 lbs.
- Frequent reaching, stooping, bending, kneeling, or crouching.
- Adequate vision and hearing to perform required duties.
