Field Registered Nurse Supervisor – Full time and Per-Diem

Position Summary
Sunshine Homecare, a Licensed Home Health Agency headquartered in New City, NY, is seeking an experienced Field Registered Nurse Supervisor to manage the care of patients residing in NYC and Westchester County. The role is responsible for delivering high‑quality nursing services in alignment with the agency's philosophy, policies, and Standards of Community Health Nursing Practice. It involves supervising field staff, developing and implementing care plans, and ensuring optimal patient outcomes.

• Develop, implement, and oversee patient care plans while supervising field staff.
• Observe, record, and report patient needs and concerns accurately.
• Coordinate home‑care services, ensuring effective communication with patients, service coordinators and other care providers.
• Maintain thorough, accurate, timely and compliant documentation.
• Meet productivity standards as defined by the Director of Patient Services.
• Monitor patient progress toward goals.
• Supervise home health aides and personal care aides in accordance with regulatory requirements.
• Maintain open communication with healthcare team members.
• Adhere to company policies and procedures.
• Complete required company and state‑mandated forms and training in a timely manner.
• Demonstrate professionalism in attitude, communication, and attire.

• Graduate of an accredited Registered Nursing (RN) program.
• Active state licensure and registration as a Registered Professional Nurse.
• A minimum of one year of home care experience.
• Knowledge of EMR systems such as HHAexchange; RN PAD helpful.

Salary
: $95,000 – $110,000
