Case Manager

The Recovery Coordinator is responsible for assessing, planning, and skill‑transferring recovery treatment and support plans for adults and youth with an Intellectual Developmental Disability and mental health diagnosis. They provide intensive therapeutic services in community living arrangements, schools, community settings, natural support residences, etc., and collaborate with team members to implement and transfer skills identified in the recovery plan using a person‑centered approach.
