The Grievance/Chronic Care nurse monitors patients from intake to the facility and ensures continuity of care leading up to and following release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach in serving the patient population in a correctional environment.
MINIMUM REQUIREMENTS- Graduate of an accredited School of Nursing
- Licensure as a LPN in the state of employment.
- Possesses an active CPR certification.
- Remains knowledgeable about specific state laws and regulations governing practice.
