Job Overview
RWJBH is seeking a Clinical Care Coordinator, Registered Nurse for our Radiation Oncology Department at the New Brunswick campus.
The Clinical Care Coordinator ensures continuity of care through clear communication on patient status with the inpatient unit staff and department staff as appropriate. This includes proactively managing patients who are in the department from an inpatient unit or patients to be admitted to the inpatient unit.
Assists with the coordination of patient care services from the inpatient setting to the ambulatory setting or vice versa by communicating with the floor nurses as needed.
Works with the office coordinator to ensure that all relevant clinical information is obtained, reviewed and available in Radiation Oncology ARIA prior to the consult and follow up visit. Works with physician to ensure follow up orders are written and carried out proactively and ensures all orders are completed and signed off according to RWJ Hospital and Department of Radiation Oncology Policy.
Responsibilities- The Radiation Oncology Nurse Coordinator is a professional Registered Nurse who assumes clinical responsibility for a specific caseload of patients.
- Supports the research mission by collaborating with the care team and monitoring patients as appropriate.
- Reviews orders and delivers care as appropriate including but not limited to administration of medication, hydration, oral/IV contrast and phlebotomy.
- Maintains continuity by following up with patients via telephone on any issues that are pending from the visit.
- Has working knowledge of current radiation oncology clinical trials and communicates to research nurse and/or physician of potential trial candidates.
- Working under the direction of a physician, the radiation oncology nurse specialist accesses the patients’ past history and current condition.
- Establishes and monitors the family goals and outcomes.
- Coordinates, monitors and evaluates patient care in the outpatient setting.
- Accesses and implements patient/family education and home care needs.
- Uses an understanding of the principles of growth and development to assess each patient’s age-specific needs and age-specific treatment and care for the pediatric, young adult, adult, and geriatric population.
Required:
- Bachelors of Science in Nursing (BSN)
- 2 years of nursing experience with a focus in ambulatory and/or oncology nursing preferred
Preferred:
- Knowledge of EMR and workflows in the ambulatory setting a plus
- OCN or MUST be obtained within 2 years of hire date
- ACLS and PALS or MUST be obtained within 2 years of hire date
Certifications And Licenses
Required:
- BLS
- Active New Jersey Registered Nurse License
