Overview
Rutgers University Behavioral Health Care (UBHC) ("UBHC"), established in 1971, offers a full continuum of evidence based behavioral health and addiction services across New Jersey. UBHC employs numerous professionals dedicated to treatment, prevention, and education.
The LPN - CSS Middlesex position provides medical-focused support services, including wellness education and support, to individuals with mental health and substance use disorder diagnoses in homes and other community-based settings, in accordance with program requirements.
Responsibilities- Participates in data collection related to the individuals' status and needs.
- Promotes health and safety through education about medication, Medication Assisted Therapy (MAT), HIV/STD, physical health promotion, and other medical topics to promote wellness and recovery.
- Completes all documentation within prescribed timeframe.
- Contributes to and supports an environment that enhances the positive self-image of individuals served and preserves their human dignity.
- Completes Annual Employee Health Update (including PPD).
- Understands and adheres to Rutgers' compliance standards as they appear in RBHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
Minimum Education and Experience: Currently licensed as a Licensed Practice Nurse in the State of New Jersey.
Certifications/Licenses: Valid registration as a Licensed Practical Nurse in the State of New Jersey. Valid driver's license.
Required Knowledge, Skills, and Abilities:
- Able to use computer to document services and enter data.
- Ability to speak clearly and write proficiently.
- Ability to focus on the task at hand despite interruptions.
Preferred Qualifications:
- Two (2) years of related experience preferred.
- Community based outreach experience preferred.
- Bilingual skills (English/Spanish) preferred.

Physical Demands and Work Environment: Provide 24/7 on-call coverage as assigned; may include evenings, weekends and holidays; field documentation in electronic medical records; community-based work across the region; transportation of individuals served in state vehicles; ability to lift up to 20 pounds; work primarily in individuals homes, shelters, soup kitchens, and locations where homeless individuals congregate; crisis intervention and potential exposure to unpredictable situations.
Subject to use of personal protective equipment for blood borne pathogens.
Special Conditions: Must possess own vehicle for outreach.
