Job Description

If you are looking for a collaborative, dynamic, physician-led practice environment where you can learn, grow, and excel in providing integrated, multidisciplinary, patient-centered care, then Summit Health / Starling Physicians is the place to be! We are currently seeking a licensed Advanced Practice Nurse (APN) or Physician Assistant to work collaboratively with our Cardiology Physicians providing patient care in the outpatient office.

Requirements include:

  • Current Connecticut State APN or PA License
  • Board Certified
  • Cardiology experience preferred
  • BLS and ACLS certification required
  • Excellent clinical judgment, communication and time management skills
  • Comfortable working both independently and collaboratively in a fast-paced outpatient environment

Lead

Responsibilities include:

  • The lead will assist the practice manager and physicians with professional development and annual reviews of APP staff as needed.
  • The lead will meet once a month with the Department Chair to discuss and resolve any pertinent section or departmental issues.
  • The lead will serve in the chain of command to receive inquiries/complaints/issues from APPs in the department, and resolve them, when possible, as well as communicating feedback from the physicians to the APPs.
  • The lead will involve leadership higher in the command chain (Director, Vice-Chair, Chair, SLD) when needed to resolve more complex issues or to obtain guidance/approval for any decisions.
  • The lead will meet monthly with APPs to provide updates as well as elicit feedback
  • APPs in the department will have access to the lead for any questions.
  • The lead will develop, maintain, and coordinate cardiology educational curriculum for APPs to highlight evidence-based practice and maintain clinical standards.
  • The lead will ensure proper training and mentoring of new APP hires, organizing their onboarding and setting up local mentors when indicated.
  • The lead will interview all APP candidates and involve other APPs for the interview process, as needed.
  • The lead will oversee schedule management and patient backlog trends, making proactive adjustments to ensure operational efficiency and maintain timely, safe patient care.
  • The lead will guide patient care initiatives and collaborate with key stakeholders to ensure effective planning, implementation, and rollout of departmental initiatives.
General Responsibilities
  • Conduct comprehensive patient assessments, including history, physical examination, and diagnostic evaluations in an outpatient setting
  • Manage and monitor chronic cardiac conditions such as heart failure, arrhythmias, hypertension, and coronary artery disease
  • Interpret ECGs, echocardiograms, stress tests, and other cardiac diagnostics
  • Perform stress tests
  • Review and manage test results, labs, and imaging; communicate findings and next steps with patients promptly
  • Provide patient advice and triage over the phone for individuals experiencing symptoms such as chest pain, palpitations, shortness of breath, or medication concerns
  • Develop, implement, and adjust treatment plans in collaboration with supervising cardiologists
