* Key Responsibilities:

*** Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.
* Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.
* Monitor, assess, and document patient progress while ensuring safety and comfort.
* Educate patients and families on treatment plans, medication management, and discharge planning.
* Support a culture of excellence, teamwork, and continuous learning.

* Qualifications:

**
* Required:

Current Registered Nurse (RN) license in State of Connecticut.

* Required:

Graduate of an accredited school of nursing (BSN preferred).

* Required:

Demonstrated competence in the clinical setting.
