**** QUALIFICATIONS**
- Required:

Graduate of an accredited RN School of Nursing;
Bachelor's degree in nursing or allied health preferred.

- Required:

Minimum At least three years of related experience with performance improvement, case management, and/or care management including at least three years of management/supervisory experience; or an equivalent combination of related education and experience.

- Required Minimum of three years leadership experience in a hospital setting,.

- Required:

Skilled in interpersonal communications, both written and oral, to deal effectively with all levels of health care providers and administration.

- Required:

Computer skills including word processing and spreadsheet programs.

- Preferred:
Previous experience managing /coordinating staff or leading a successful project Implementation.
** JOB SUMMARY**- Has primary responsibility for assessing that clinical documentation required for regulatory and performance improvement activities are established and documented during the patient’s course of care. Provides guidance for optimizing team resources and patient specific outcomes within best practice, ethical, legal, and regulatory parameters. Serves as a central resource for the Clinical Teams concerning strategies to assure effective, timely and patient-centered care documentation that supports continuity and comprehensive care planning.

Identifies risks as it pertains to regulatory/accrediting environment through various auditing and review procedures. This may include examination and documentation of clinical activities in medical records, review and survey of patient and the care environment as well as other programmatic activities of the clinical system, utilizing both manual and computer-assisted techniques. This role serves to facilitate an interdisciplinary approach to patient care.

Collaborates with other departments and activities within the hospital working in coordination with the necessary clinical department heads of the hospital. This position is responsible to assure that the systems and analysis required by regulatory and accreditation standards are in place. Collaborative working relationships will be maintained with medical records and fiscal departments, and other departments as needed. The Quality Coordinator/Auditor is expected to facilitate the implementation of corrective action plans.

** PHYSICAL DEMANDS**- Job requires standing and/or walking for long periods, up to one hundred fifty minutes at a time with infrequent sitting. Overhead reaching, climbing, squatting, and kneeling are occasionally required. Bending and forward reaching frequently required to access equipment from high and low areas. Lifting of weights greater than fifteen pounds is occasionally required. Pushing of objects greater than fifty pounds is occasionally required.
** COGNITIVE DEMANDS**- Job requires problem identification and thoughtful decision-making is required. A high degree of attention to detail, discernment, perception and/or concentration is necessary.

- Job requires legibility, speaking accuracy and logical sentence and paragraph organization. A working knowledge and familiarity with professional (medical) terms and definitions is required.

- Job requires a high level of communication skill, active listening and an ability to dialogue with multiple parties.

- Job requires an ability to comprehend abstractions and to make inferences. A working knowledge and familiarity with professional (i.e medical) terms and definitions required.

** WORK DEMANDS**- Job requires working primarily indoors. Job requires working continuously around others while working alone and with others on a frequent basis. Flexibility in scheduling of hours may be required.
** ESSENTIAL FUNCTIONS**- Works closely with medical staff, patient care team members to serve as a resource to facilitate effective care planning, care provision documentation. Serves as mentor, promoting clinical staff to foster best practices for clinical documentation and professionalism.

- Primary responsible party for the hospital’s compliance with the mandated CMS Quality Reporting Program data collection. Maintains working knowledge of all required indicators for submission. Works to understand and interpret new indicators proposed and implemented.
