Position Summary

Empowers clients by providing ongoing support, guidance, supervision, monitoring and role modeling. Facilitates client improvement in the areas of personal responsibility, social skills, community living skills and behavior.

Responsibilities
  • Serve as key staff member for clients assigned to you, assuring supervision and accountability for them at all times.
  • Be available to fill staff vacancies on an as needed basis.
  • Provide and assist with development of treatment and/or service plans, goals, and individual counseling to client to discuss interpersonal issues and treatment issues.
  • Demonstrate the ability to assess client skill levels and counsel clients in the areas of independent living, coping strategies, pro-social, interpersonal, conflict resolution, and navigating peer pressure.
  • Maintain professional boundaries and adhere to limits.
  • Ensure client safety is a priority.
  • Ensure client success by implementing an effective daily schedule that inspires clients to achieve goals.
  • Role model strong interpersonal skills, pro-social behaviors and professional responsibility to internal and external community.
  • Demonstrate computer literacy by utilizing an electronic health record for accurate documentation and an HRIS for hours and time-off requests; utilize email.
  • Attend and participate in staff meetings.
  • Participate in and facilitate program group sessions when appropriate.
  • Prepare summaries of interactions and objective behavioral observations of clients at the end of each shift.
  • Participate in staff training, maintain certifications as required, and pass proficiency tests where appropriate.
  • Provide safe transportation for clients as necessary.
  • Perform crisis prevention and interventions including de-escalation techniques and physical management as needed and allowed.
  • Complete other related duties as assigned and perform all duties in accordance with the Agency’s policies and procedures.
Desired Qualifications
  • High School Diploma (Required)
  • Nice to have associate or bachelor’s degree
  • 1–2 years of relevant experience in Social Work, Healthcare, Nursing, Human Services or related field
  • Strong communication and organizational skills
  • Ability to work independently and as part of a team
  • Commitment to maintaining professional boundaries and ethical practice

Pay: $19.50 per hour
