**** QUALIFICATIONS

* Required:

M.S.W. from an accredited graduate school of social work.
* Current licensure by the State of Connecticut as a Clinical Social Worker (LCSW).

* Required:

Competence in meeting the needs of patients who require treatment for chronic disease, complex medical issues and/or rehab therapies.
* Preferred:
One-year clinical experience in a medical setting.
** JOB SUMMARY
*** The Outpatient LCSW works collaboratively with the interdisciplinary team, to assess the needs of patients and their family/caregivers when appropriate, and arranges, coordinates, and advocates for services to meet the members physical and psychosocial needs. The social worker provides education to staff, patients and families as necessary
** PHYSICAL DEMANDS
*** The job requires carrying of documents, books, and materials; occasionally the transport of patients, frequent walking, standing, sitting during many counseling/therapy sessions or meetings usually no more than ninety minutes; reaching and bending to access file drawers or cabinets, possible participation in physical restraint of patients, lifting and carrying of children. Manual dexterity is required to write type or use the computer and to access the dictation system.
** COGNITIVE DEMANDS
*** Job requires independent problem identification. Abstract reasoning and adaptability to new situations, changing environments and information and quick decision making are required. A high degree of selective attention, discernment, perception and/or concentration in the presence of noise/distraction is necessary.
* Job requires the ability to analyze and synthesize information, to discriminate relevant from irrelevant information, to determine prognosis through interpretation of information, to learn new techniques and apply therapy to functional context, reading, auditory comprehension of abstraction and an ability to make inferences.
* Job requires a high degree of writing proficiency including attention to detail, adherence to all elements of style and ability to postulate. It also includes a working knowledge and familiarity with medical terms and definitions.
* Job requires a high level of communication skill including expression of abstract/complex ideas, use of negotiation and active listening skills. This includes an ability to dialogue with multiple parties. Also, to modify communication skills to the listening level, ability or readiness to comprehend.
* Job requires the ability to add, subtract, multiply, divide with all units of measure, and to perform arithmetic operations involving all American monetary units.
** WORK DEMANDS
*** Job requires primarily working indoors with an occasional need to be outdoors. Job requires working around many other people and the ability to interact with a variety of disciplines on a continuous basis. Schedule fluctuation is frequent and is often unpredictable due to census, documentation requirements and/or program development. There is exposure to body fluids, contagious disease and occasionally to blood-borne pathogens and radiation.
** ESSENTIAL FUNCTIONS
*** Establishes and maintains effective working relationships with community providers, social service agencies and members of the interdisciplinary team. Maintains flexibility and adjusts approach to the needs of the team.
* Researches and arranges community resources for patients/caregivers.
* Provides supportive counseling to patients/families to address psychosocial issues that may affect the plan of care established by the team.
* Runs patient group meetings specific to disease or identified psychosocial needs to support the outpatient population.
