Care Coordinator, Continuing Care

Job Summary

The Care Coordinator works to improve patient outcomes and satisfaction by coordinating and facilitating patient care across a multidisciplinary care continuum while ensuring compliance with associated requirements. Coordinates assessment, planning, and implementation of health care and related services to assigned patients. Patient age groups range from infant to old age, or death.

  Nursing;or Bachelor’sDegreein Social Work;or Bachelor’sDegreein Human Services /Social Services field with 3-5 years relevant hospital social service work experience.
• If nursing degree, must have current N.C. license.
• Three to 5 years of experience in acutehealthcaresettingpreferred.
• Possesses the ability to relate effectively to patients, their families, physicians,staffandallothers,usingproperchannelsofcommunication.
• Ability and willingness to perform all job functions in compliance with Carolina East Health Systemandregulatorysafetystandardsandinsures  thesafetyofpatients,self,visitorsandotheremployees.
• Must be able to demonstrate the knowledge and skills necessary to provide services according to the age of the patient population served.
• The individual must demonstrate knowledge of the principles of growth anddevelopmentoverthelifespanandpossesstheabilitytoassessdata reflective of the patient's status and interpret the appropriate information neededtoidentifyeachpatient'srequirementsrelativetohis/herage.
