Community Health Technician - Hospice (10079)

The primary purpose of this position is to provide supportive activities including personal care and assistance with activities of daily living in accordance with a plan of care, signed by the Hospice Medical Director and the recipient's Physician of Choice. The provision of personal care is a constantly changing need that changes as a person's life limiting illness or condition deteriorates and progresses.

The Community Health Technician is to report any and all changes to the RN Case Manager so adjustments to care plan can be made as needed.

• Patient Care:
  • Bathes and assists with personal hygiene (bed, tub, or shower).
  • Prepares meals and serves/feeds patient, as ordered or needed.
  • Maintains general cleanliness of patient's room and bathroom.
  • Administers enemas as needed and ordered, suppositories and assists with bedpans and urinals.
  • Assists with oral hygiene, care of hair, nails, and feet.
  • Gives attention to patient positioning in the bed and provides care for the skin.
  • Assists with wound care and non-sterile dressings when instructed.
  • Assists with physical activity program as needed and ordered, which includes assisting with range of motion exercises, ambulation, and transfers.
  • Makes patient bed and changes linens as needed and ordered.
  • Assists with post-mortem care as needed.
• Travel:
  • Travel to and from patient homes
• Administrative / Additional Support:
  • Assists RN Case Manager with maintaining supplies in the home and helps keep supply room replenished and in order.
  • Completion of agency training as required and 12 hours of in-service training each year
  • Other duties as assigned by supervisor and/or management
  • In the event of a disaster, may be required to assist as needed.

Demonstrated possession of knowledge, skills, and abilities as stated through at least two years of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, or medical treatment program.

• Valid Driver's license.
• Listed with North Carolina Division of Facility Services as Nurse Aide I or II.
• CPR Certification.
• G.S. 153A-94.2(b) Subject to SBI criminal history record check prior to hire.

• Ability to provide basic health related services and to follow instructions as outlined in the personalized care plan of each patient.
• Ability to safely lift and transfer patients without injury to the patient or self.
• Ability to safely drive an automobile in all kinds of weather, under differing road conditions, in order to reach patient homes to provide personal care.
• Ability to use a variety of Durable Medical Equipment including electric hospital beds, hospital bed side rails, walkers, wheelchairs, and mechanical lift devices as needed and ordered.

Physical Requirements:

The position requires physical demands of lifting, transferring and pushing patients in wheelchairs, bending and reaching to complete tasks without injury to oneself or the patient.
