Job Summary

The Care Coordinator works to improve patient outcomes and satisfaction by coordinating and facilitating patient care across a multidisciplinary care continuum while ensuring compliance with associated requirements. Coordinates assessment, planning, and implementation of health care and related services to assigned patients. Patient age groups range from infant to old age, or death.

Minimum Requirements
  • Associate’s Degree or Bachelor’s Degree in Nursing, or Bachelor’s Degree in Social Work, or Bachelor’s Degree in Human Services/Social Services field with 3‑5 years relevant hospital social service work experience.
  • If nursing degree, must have current N.C. license.
  • Three to 5 years of experience in acute healthcare setting preferred.
  • Possesses the ability to relate effectively to patients, their families, physicians, staff, and all others, using proper channels of communication.
  • Ability and willingness to perform all job functions in compliance with Carolina East Health System and regulatory safety standards and ensures the safety of patients, self, visitors and other employees.
  • Must be able to demonstrate the knowledge and skills necessary to provide services according to the age of the patient population served.
  • The individual must demonstrate knowledge of the principles of growth and development over the lifespan and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age.
  • Deliver outstanding customer service that upholds Carolina East's Standards of Excellence.
