Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

We are seeking a Therapeutic Behavioral Specialist to join our team! You will provide recovery-oriented support and case management services to residents of long-term care facilities and their families. Opportunities may exist for a future transition to providing counseling services. We care for individuals in long-term care settings, addressing issues from anxiety and depression to severe mental illness and end-of-life concerns.

As part of a multidisciplinary team, our clinicians can significantly improve residents' quality of life by solving problems, delivering excellent clinical care, and serving as a vital link in their care.

Essential Duties and Responsibilities:

  • Provide Therapeutic Behavioral Services by consulting with a licensed clinician to meet individual needs.
  • Develop service plans for supports or care coordination, including healthcare and behavioral health treatment plans.
  • Implement linkage, interventions, treatment options, daily life skills restoration, and crisis prevention services.
  • Deliver services to individual clients or groups.
  • Train support staff on client triggers, de-escalation, and interventions as per the treatment plan.
  • Foster positive relationships with outside contacts.
  • Meet with Resident Care Coordinators or staff upon facility visits to review resident and care needs, communicating findings or concerns.
  • Maintain open communication with facility staff and management, providing verbal and written feedback on treatment plans and caregiver information.
  • Adhere to Eventus Whole Health standards, policies, procedures, and all legal and regulatory guidelines, including HIPAA.
  • Conduct oneself professionally at all times.
  • Maintain patient confidentiality and appropriate use of communication tools and records.
  • Consult with the clinical supervisor when encountering unfamiliar residents or facility requests.
  • Qualifications/

    Education:

    Master’s degree in counseling or marriage and family therapy, with eligibility for licensure in the practicing state.

    Knowledge:

    • Population-specific interventions and care following treatment plans.
    • Rules and regulations of behavioral health practice governing bodies.
    • Organizational policies and procedures.

    Skills:

    • Data gathering and analysis of physiological, socioeconomic, behavioral, and emotional patient information.
    • Training in social/emotional well-being, stress, anger management, and life skills.
    • Accurate evaluation and appropriate intervention or referral skills.
    • Strong written and verbal communication skills.
    • High initiative, judgment, and discretion.
    • Effective relationship-building abilities.

    Abilities:

    • React calmly and effectively in emergencies.
    • Collaborate with healthcare team members.
    • Evaluate and recommend quality improvements.
    • Handle sensitive, confidential information responsibly.
