Horizon's Inpatient Hospice facility, The Kathy Hospice, located on the Froedtert West Bend campus, practices Horizon's hospice philosophy of celebrating the human spirit and delivering quality and compassionate care to patients and their families.

In Horizon's Hospice Inpatient Units, you will play an important role in supporting and educating patients and their families as they embark on the end-of-life journey. You will draw on your science knowledge, skills from experience, and above all, compassion and empathy. We are seeking experienced nurses to join our Hospice Inpatient team.

• Perform referral evaluations / admission assessments of patients and families seeking Hospice services.
• Conduct physical, psychosocial and spiritual assessments utilizing non-verbal cues and probing to obtain comprehensive information.
• Communicate patient and family expectations of care, goals and learning needs to all members of the health care team via appropriate documentation.
• Ensure all documentation is clear, legible, concise, accurate, timely, and meets department standards.
• Provide Case Management functions as part of the IDG to an assigned caseload.
• Provide services at quality levels consistent with professional standards of practice; approach patients and families with care, concern and compassion.
