Responsibilities

POSITION SUMMARY: The Recovery Coach meets with newly enrolled individuals after their screening review to assist with the enrollment process and begin developing therapeutic rapport to support their addiction recovery. As part of the Integrated Team, the role assists individuals in identifying meaningful goals for their Individual Action Plan/Treatment Plan, focusing on areas such as mental health recovery education, daily living skills, recovery from addictions, relationships, vocational activities, and leisure.

Up to $24/hr, negotiable

DUTIES AND RESPONSIBILITIES:

  • Meet newly enrolled individual’s after screening is reviewed, to assist with enrollment process and begin to develop therapeutic rapport.
  • As part of the Integrated Team, assist individual’s with identifying meaningful goals for their Individual Action Plan/Treatment Plan focusing on the following areas: mental health recovery education, daily living skills, recovery from addictions, relationships, vocational & leisure.
  • In conjunction with the individual and Integrated team develop a comprehensive Individualized Action Plan (IAP)/Treatment, per Program Regulation. Ensures a multidiscipline approach is taken, using evidence-based practices which support recovery.
  • Practice a trauma informed approach to treatment planning and service delivery.
  • Work with individuals through life transitions and collaborates with individual and Integrated Team to develop the Crisis Management Plan.
  • Integrate the PRISMModel® for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.
  • Promote integrated services that meet the complex needs of individual's recovery goals
  • Develop interventions that encompass all the individual's life domains including but not limited to the following:
    • Physical and psychological wellness
    • Activities of daily living
    • Safe Housing
    • Interpersonal skills and relationship building
    • Employment and community connectedness
    • Cultural and spiritual
    • Arts and Wellness
  • Facilitates peer lead activities, empowering enrolled individuals to take a lead in the planning and implementation process
  • Develops, in conjunction with peer lead groups, information related to wellness and recovery
  • Engages in non-judgmental, compassionate active listening and shares personal recovery experiences to build peer relationships
  • Provides peer counseling, support and education to validate individual's' life experiences.
  • Works with individuals to identify and utilize preferred recovery tools and self-care techniques (i.e. WRAP, peer groups).
  • Utilizes motivational interviewing techniques.
  • Encourages individuals use of and involvement in the Recovery Learning Community, where applicable.
  • Supports participation in individual's self-help & advocacy organizations that promote recovery.
  • Provides training and consultation to team to facilitate understanding individual's' perspective and protection of individuals rights.
  • Peer encourages on-going engagement and involvement of family members in all phases of service delivery and evaluation based on the desires of the individual this may include ensuring family members understand:
