Case Manager – Part Time

High Point & Affiliated Organizations is hiring a Part‑Time Case Manager to work with clients in the Transitional Support Services (TSS) program in New Bedford.

Program & Location

Transitional Support Services (TSS), New Bedford

Education & Experience

Minimum of a High School Diploma or equivalent. Associate’s degree or higher preferred. Supervised counseling experience in substance use treatment or a closely related field is preferred.

Pay Range

Diploma (Clinician
3): $22 (experience/education dependent)

Bachelors/LADC 2 (Clinician
2): $28 (experience/education dependent)

Masters (Clinician
1): $34 (experience/education dependent)

Schedule

Part Time – Saturdays and Sundays: 8 am – 4:30 pm or 8:30 am – 5 pm

Hours:

16 hrs per week

About Us

Our Transitional Support Services Programs (TSS) bridge the gap between detox and step‑down services to residential after‑care. High Point & Affiliated Organizations is a health and human service agency that treats and prevents substance use disorders and mental illness. We have programs throughout southeastern Massachusetts offering inpatient, outpatient, residential, and community‑based services. We also assist survivors of abuse, violence, and families experiencing homelessness.

Case Manager Requirements
  • Maintain a courteous and professional appearance at all times
  • Demonstrate flexibility and the ability to perform multiple tasks
  • Have excellent communication skills, organizing ideas logically and clearly
  • Possess basic computer skills and familiarity with common software programs
  • Work collaboratively with others
  • Understand the relationship between substance use and mental health issues and/or self‑help programs
  • Maintain therapeutic boundaries
  • Reinforce rules respectfully
  • Comply with federal regulations (42 CFR Part 2, confidentiality of substance use disorder and HIPAA)
  • Complete TB/Flu and COVID‑19 assessment
  • Hold certification in an HPAO‑approved de‑escalation program
  • Hold CPR certification
  • Successfully complete all required training upon hire and annually as required
Case Manager

Duties & Responsibilities
  • Maintain safety and security of residents and the facility
  • Ensure timely contact of Program Manager/authorities during crisis or emergency
  • Follow referral procedures and schedule intake appointments
  • Facilitate intake of new residents, set up new records, and complete intake paperwork
  • Complete Initial Individual Service Plan (ISP) with resident
  • Provide referrals to needed services (e.g., DTA, SSA, DCF, medical/mental health agencies)
  • Maintain accurate case files, secure necessary releases, prepare clear case‑management notes, document referrals, update ISPs monthly, and ensure timely periodic evaluations
  • Accurately record resident attendance at meetings, appointments, groups, etc.
  • Meet with each resident bi‑weekly to review and evaluate progress, revise or update the ISP, and establish new goals and action steps
  • Attend monthly case reviews with Program Manager
  • Attend clinical supervision, staff meetings, and trainings
  • Ensure timely completion of admission/discharge satisfaction surveys
  • Provide house‑driving/transportation as needed
  • Perform other duties assigned by Program Manager
Case Manager Qualifications
  • High School Diploma or GED and one year of experience in substance use treatment is preferred
  • Valid driver’s license
