Starting at $56,160 based on experience.
Essential Duties- Collaborate with professionals to evaluate patients' medical or physical condition and assess client needs.
- Refer patients, clients, or families to community resources to assist in recovery from mental and physical illness and provide access to services such as financial assistance, legal aid, housing, job placement or education.
- Counsel residents and families to help them overcome dependencies, recover from illness, and adjust to life.
- Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
- Advocate for residents to resolve crises.
- Collaborate with residents, families, and staff regarding the development of an appropriate admission and discharge plan of care.
- Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient.
- Modify treatment plans to comply with changes in clients' status.
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
- Identify environmental impediments to client patient progress through interviews and review of patient records.
- Investigate abuse or cases and take authorized protective action when necessary.
- Conduct social research to advance knowledge in the social work field.
- Participate in care conferences, consulting with the team caring for the resident, to evaluate the resident's needs and plan for continuing services.
- Attend and participate in all appropriate meetings and training opportunities to keep informed on current information and skill development.
- Follow established policies and procedures, including but not limited to Presbyterian Villages policies and procedures.
Bachelor's Degree in Social Work or related field.
ExperienceNone required. Experience in a geriatric environment is beneficial and preferred.
Computer Skills- Proficient in operating a computer
- Proficient in Microsoft Office Software
MI Social Worker License.
ScheduleMonday - Friday.
