Responsibilities- Assist residents and families in adjusting to physical, personal psychological and environmental difficulties which may interfere with quality of life.
- Provide guidance and advice to medical and administrative personnel regarding the impact of psychosocial factors on the treatment and progress of residents.
- Advocate for residents to resolve crises.
- Collaborate with residents, families and staff regarding the development of an appropriate admission and discharge plan of care.
- Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
- Counsel residents and resident families to help them overcome dependencies, recover from illness, and adjust to life.
- Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the client or patient.
- Modify treatment plans to comply with changes in clients status.
- Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
- Identify environmental impediments to client or patient progress through interviews and review of patient records.
- Investigate abuse or neglect cases and take authorized protective action when necessary.
- Conduct social research to advance knowledge in the social work field.
- Participate in care conferences, consulting with the team caring for the resident, to evaluate the resident s needs and plan for continuing services.
- Attend and participate in all appropriate meetings and training opportunities to keep informed on current information and skill development.
- Bachelor s Degree in Social Work or related field
- Experience:
None, but experience in a geriatric environment is beneficial and preferred - Computer
Skills:
Must be able to operate a computer;
Proficient in Microsoft Office software - Certificates & Licenses: MI Social Worker License
