- Assist residents with activities of daily living (ADLs) such as bathing, grooming, mobility, and meals
- Support nursing staff in monitoring and reporting changes in resident health
- Promote a safe and comfortable environment for residents
- Build positive relationships with residents, families, and your team
- Receiving and following written or verbal care assignments and providing daily care of residents, including:
- Assisting residents with bathing, toileting, oral hygiene, skin care, grooming, nail care, and dressing needs
- Feeding residents, providing bedside water, snacks, and transferring them to and from dining areas as needed in compliance with nutritional plans
- Checking catheters frequently and providing catheter care as assigned by the nurse
- Assisting bedfast and comatose residents in turning, positioning, exercising, transferring, and ambulating as per facility protocol
- Answering resident call lights and responding promptly to assistance requests in accordance with facility policy
- Maintaining resident rooms—making beds, changing linens, cleaning bedside tables, and organizing personal belongings as needed
- Observing and reporting any changes in appetite, skin condition, or behavior that deviate from’équilibrium, according to facility policy
- Offering emotional support and actively listening to residents to build trust and independence in daily living
- Maintaining confidentiality of all residents and their care, ensuring privacy and rights are protected at all times
- Reporting and documenting incidents, accidents, complaints, or allegations using the designated reporting system
- Providing medical record documentation in accordance with facility policy, including:
- Taking and monitoring vital signs, intake and output, weights, and meal consumptionsટર as assigned by staff nurse
- Complying with documentation requirements for functional maintenance plans, 24‑hour reports, CNA flow sheets, monthly intake logs, BM flow records, behavior management records, toileting plans, shower lists, skin care audits, cleaning schedules, protective device usage, and other assigned records
- Complying with safety requirements, infection control measures, and maintaining a clean work environment, which involves:
- Using protective devices such as side rails, geriatric chairs, and poseys appropriately and under nursing directionиб
- Abiding by emergency protocols for crash carts, resident codes, fire safety, missing residents, tornado preparation, violent residents, wander guard alarms, door alarms, and other facility policies
- Reporting any suspected abuse immediately to the nurse manager
- Following infection control and universal precautions procedures
- Adhering to periodic cleaning schedules for equipment and returning equipment to proper storage after use
- Ensuring nursing service work areas remain clean and sanitary
- Performing other duties as assigned by staff nurse or designee

- A high school diploma or GED (unless working under a student permit)
- Certified by the Indiana State Department of Health (ISDH) as a C.N.A. in good standing. CNAs transferring from another state may work for 120 days while awaiting Indiana certification, and graduating CNA students not yet certified may also work for 120 days while awaiting…
