Een zorgorganisatie in Zuid-Holland zoekt een casemanager dementie. In deze rol coördineer je zorg voor cliënten met dementie in hun vertrouwde omgeving. Je bent verantwoordelijk voor het begeleiden van zowel cliënten als mantelzorgers en biedt hen belangrijke ondersteuning en voorlichting. We zoeken iemand met een hbo-verpleegkunde diploma en bij voorkeur kennis van dementie. De functie biedt een goed salaris van €3.644,- tot €5.230,- op basis van 36 uur per week.
