Role Purpose:
The Manager of Interprofessional Primary Care leads and supports a collaborative, interprofessional team in delivering comprehensive community-based health services, including primary care, hospice palliative care, health promotion, and sexual assault/domestic violence programs. This role focuses on operational excellence, program development, and high-quality, person-centred care.
Key Responsibilities:
Leadership & Interprofessional Care
- Champion integrated, evidence-based primary care and person/family-centred approaches.
- Lead interprofessional teams, drive organizational change, and expand primary care services across partner sites.
- Collaborate with the Director to evaluate program outcomes and identify opportunities for new programs, service improvements, and operational efficiencies.
Team Development & Orientation
- Supervise, mentor, and support professional development of staff, students, and volunteers.
- Ensure compliance with legislation, standards of practice, and interprofessional team guidelines.
- Participate in task forces, committees, and interprofessional initiatives.
Operational & Administrative Management
- Support implementation of team work plans and accreditation standards.
- Oversee scheduling, on-call rotations, and workflow redesign for efficiency.
- Lead investigation and resolution of client complaints.
Budget & Planning
- Monitor budgets, perform variance analysis, and forecast financial needs.
- Develop and evaluate annual operational plans aligned with strategic goals.
- Identify risks and implement mitigation strategies to ensure program sustainability.
Qualifications & Experience:
- Undergraduate degree in Health Sciences or related field;
Masters preferred. - Clinical leadership experience is beneficial.
- 3-5 years managing interprofessional, culturally diverse health teams.
- Experience in staff supervision, HR planning, recruitment, and community health program management.
Skills & Attributes:
- Clinical leadership, strategic thinking, strong communication, and interpersonal skills.
- Commitment to integrated care and social determinants of health.
- Ability to foster collaboration, high morale, and team growth.
- Proficiency with EMRs (PSS) and computer applications.
- Valid Ontario drivers license and access to a vehicle.
Working Conditions:
- Evenings/weekends as needed, and travel across program sites.
- Remuneration:
Commensurate with experience.
