Description
Services Coordinator:
Early Childhood
Monday-Friday, 20-29 hours/week, no on-call.
May travel in the northern and central panhandle area. May be based out of Chadron or Alliance office. Will depend on candidate preference.
Primary Functions:Services Coordinator is responsible for identification, referral and coordination of services for adults, youth, and/or children and families. The services coordinator also serves as the single point of contact in helping clients gain needed services and assistance.
- Secures information releases to facilitate sharing of information.
- Maintains ongoing communication with referral source and other contacts as requested by the individual or family.
- Identifies individual or family needs, strengths, and priorities on a continuing basis.
- Assists the individual or family in forming the support team and gaining access to services.
- Maintains current information base regarding services available within the community.
- Develops and maintains interagency contacts.
- Completes, updates, and disseminates case plan documents, as appropriate, following confidentiality and time frame guidelines.
- Facilitates problem solving and collaboration of community service providers around changing needs and assists in modifying the case plan documents.
- Serves as mediator for addressing and managing conflicts among individuals and/or families, agencies, and service providers.
- Facilitates the development of Individual Action Plans/goals, therapy needs, transition plans, and safety plans.
- Maintains records of contacts with individual agencies, and/or families, including meeting summaries to include progress and ongoing concerns.
- Maintains required data and confidential records.
Education:
- Preferred - Bachelor's Degree or higher in psychology, sociology, social work, or related human service field.
- Two years' in a health and/or human services field or 24 semester hour of post high school education in behavioral science, social science, or closely related field.
- Experience with a broad variety of community agencies that provide services to adults, youth and/or families and young children with disabilities
- Experience working with culturally diverse individuals and/or families.
- Valid Driver's License
