The Long Term Care (LTC) Certified Nursing Assistant (CNA) serves as caregiver to the resident during the scheduled work period in long term care, providing resident‑centered nursing care and daily living assistance under the supervision of an RN. The CNA delivers age‑appropriate care related to the physical and psychological needs of the resident as per the care plan, follows infection prevention and control policies, and uses personal protective equipment.

Provides assistance with basic healthcare needs including daily living activities such as bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support. Assists residents with transfers, repositioning, and walking using appropriate techniques and equipment, and performs range‑of‑motion and passive exercises. Completes food preparation and dining tasks, creates a positive dining experience, supports meal and snack provision, and provides water and supplements.

Aids providers and nursing staff with procedures as needed, documents resident interactions and outcomes, communicates changes in condition and care concerns to the charge nurse, collects non‑invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services, assists residents with maintaining living areas, supports wellness and recreational programs, and encourages participation. May participate in departmental/organization committees and task forces as needed.

• High school graduate or GED preferred (not required).
• Minimum age 16.
• Prior clinical or nursing assistant experience in long‑term care preferred.
• Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of starting the position, or enrolling in a training program or in the process of obtaining certification through endorsement.
• Completes yearly department‑specific competency testing and maintains continuing education credits; minimum 12 hours annually.
• Maintains all department‑specific required certifications. When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
• Depending on responsibilities, may classify as a Sanford Category II Driver; requires motor vehicle report, valid driver’s license, and medical qualification per Sanford Driving Policy.
