Hiring For The Assertive Community Treatment (ACT) teamLutheran Family Services is forming a new team in Fremont, Nebraska. The Assertive Community Treatment (ACT) team utilizes a multidisciplinary, team-based service delivery model that offers continuous, community-based support to individuals with serious mental illness, particularly those with concurrent substance use issues, frequent hospitalizations, risk of homelessness, involvement with the criminal justice system, and psychiatric crises.
ACT is where care meets the community. Team members engage with individuals in various settings, including the office, homes, streets, shelters, and community spaces. This approach fosters genuine relationships while providing accessible, consistent, compassionate, and deep human support.

Community Response Specialist (Temporary Six-Month Position)
Job TypeTemporary Six-Month Position - Full-Time
Position Summary- The Community Response Specialist provides short-term support for Tyson Emergency Response efforts and the LFS Emergency Fund. This position assists individuals with registration, intake, documentation, and connection to appropriate community resources, with a focus on former Tyson employees seeking assistance. The Specialist helps manage increased program demand, ensure timely service delivery, and maintain high-quality, client-centered support during emergency response efforts.
- Provide direct assistance to assigned clients navigating available services and resources.
- Support registration and intake processes for individuals seeking Emergency Relief assistance.
- Work with clients to identify employment and community resource opportunities.
- Provide or coordinate interpretation services as needed.
- Maintain client confidentiality and agency best practices at all times.
- Document client information, case notes, and required data in appropriate systems in a timely manner.
- Assist with documentation and workflow needs related to the LFS Emergency Fund.
- Ensure adherence to funder, program, and organizational requirements.
- Participate in regular supervision meetings and case consultations.
- Attend required meetings and trainings.
- Consult internal resources and leadership when addressing complex client situations.
- Perform other duties as assigned.
- Strong verbal and written communication skills.
- Effective organizational and time management skills.
- Ability to maintain professionalism, integrity, and confidentiality.
- Proficiency in Microsoft Office Suite or similar software.
- Ability to work effectively with individuals experiencing crisis or sudden job loss.
- Ability to adapt to changing program demands in a fast-paced environment.

- High school diploma or GED required.
- Experience providing support to and/or working with diverse populations preferred.
- Former Tyson employee preferred but not required.
- Bilingual in English and another language preferred (languages reflective of client populations, including but not limited to Spanish, Karen, Pashto, Dari, Burmese, Somali, Arabic, or Russian).
- Prolonged periods of working at a computer and using general office equipment.
- Ability to work in fast‑paced situations during peak emergency‑response periods.
