Social Worker, Family Advocacy & Support Services

Assessment & Planning:
Meeting individuals and families to assess needs, strengths, and risks to create, implement, and review care plans.

Safeguarding:
Identifying, reporting, and taking action to protect vulnerable children and adults from abuse or harm.

Support & Advocacy:
Providing emotional and practical support, acting as an advocate to ensure clients’ rights are respected.

Case Management:
Maintaining accurate, up-to-date records, preparing detailed reports, and participating in multidisciplinary team meetings.

Intervention:
Working with individuals to develop independent living skills and navigate crises.

Collaboration:

Working with health professionals, schools, police, and courts to provide holistic care
