About the Transition Team

The Transition Team supports disabled young people as they prepare for adulthood, ensuring their journey is planned, coordinated, and rooted in what matters to them. We work collaboratively with young people, their families, Children’s Services, health colleagues, schools and colleges, and a wide range of community partners.

Our practice is guided by our mission:

“To support disabled young people to experience a smooth and empowering transition to adulthood by promoting independence, dignity, self‑determination, and inclusion.”

Every aspect of our work reflects this commitment - placing each young person’s aspirations, strengths, and rights at the centre of decision‑making.

Transition Social Work is a rapidly developing area of practice, bringing unique opportunities to blend skills from both children’s and adults’ fields. Whether your background is in children’s social work, adult services, or both, you will find your experience highly relevant and valued here.

Why Join Us? A Team Built on Innovation

We are proud to be an evolving service that embraces new ideas and creative approaches. Our work encourages professional curiosity, reflective thinking, and solution‑focused practice - making this an ideal environment for social workers who want to contribute to shaping a modern, ambitious transition offer for young people.

You will receive a warm welcome from a team known for its compassion, camaraderie, and willingness to support one another. We work in a profession that can be challenging, so we intentionally create a culture where people feel safe to learn, share concerns, celebrate successes, and ask questions.

A Strong Learning Environment

We are committed to nurturing the professional development of every team member. You will have access to:

  • Regular reflective supervision
  • Practice development sessions
  • Case mapping and multi‑disciplinary case discussions
  • Support from Consultant Social Workers and management team
  • Opportunities to develop specialist knowledge in areas such as neurodiversity, mental capacity, legislation, and strengths‑based practice

For those seeking broader career development, there are potential pathways into Practice Education, management and leadership development.

A Focus on Relationships and Realistic Caseloads

Our approach prioritises meaningful engagement with young people, ensuring you have the time and space to build strong, trusting relationships. We work hard to maintain manageable caseloads and provide the tools you need to deliver high‑quality, person‑centred support.

Opportunities to Meet Us

Applicants are welcome to arrange an informal discussion or visit with members of the team to get a real sense of our approach and culture before applying. We know that applicants may be new to this practice area, so we are happy to provide information about who we are and our way of working.
