About the Transition TeamThe Transition Team supports disabled young people as they prepare for adulthood, ensuring their journey is planned, coordinated, and rooted in what matters to them. We work collaboratively with young people, their families, Children’s Services, health colleagues, schools and colleges, and a wide range of community partners.
