Hospice RN Case Manager

Hospice RN Case Manager / Registered Nurse – Health Care Support is seeking a Hospice RN Case Manager / Registered Nurse to fill an opening with a prestigious Hospice organization in Tennessee.

Location:

Dickson / West Nashville area.

• Conduct home visits or facility visits to provide patient care.
• Collaborate with an NP, RN team, social worker, and other case managers to promote ongoing care.
• Provide wound care, monitor vitals, administer medications, and perform pronouncements of death as needed.

Monday – Friday, 8:00am – 5:00pm. On-call one night per week from 5:00pm–11:00pm (no weekends).

• Active RN License in Tennessee
• At least 2 years of RN experience
• Hospice or Palliative care experience
• Related experience in Hospice or Home Health or Oncology or ICU or Med/Surg or LTC/SNF

Pay: $74,173 - $84,886 per year (based on experience) + bonus eligibility
