Medical Social Worker- Home Health - Part Time - Smyrna

Overview

We are seeking a Medical Social Worker (MSW) for our Home Health team in Smyrna, TN, to provide emotional support, counseling, and resource coordination to patients receiving home care.

• Assist patients and families with social, emotional, and financial concerns, promoting effective coping skills.
• Deliver social work services aligned with the plan of care and communicate patient status changes to the healthcare team.
• Educate patients, families, and team members on social work interventions and treatment plans.
• Perform care planning, transfer, and discharge activities.
• Identify and address risks in the patient s environment to ensure safety and well-being.

Must Have

• Master s degree (MSW) from an accredited school
• Current unencumbered Social Work license
• At least 1 year of healthcare social work experience
• CPR certification
• Driver s license and dependable transportation
• Part-time role involving travel within the service area
