** Key Responsibilities
*** Provide high-quality, evidence-based physical therapy in patients’ homes
* Deliver care according to individualized treatment plans developed by supervising PTs
* Promote patient safety, mobility, and functional independence
* Educate patients and caregivers on exercises, mobility strategies, and home safety
* Accurately document clinical notes and patient progress in EMR systems
* Communicate effectively with interdisciplinary team members
* Travel independently to patient homes within designated service areas
** Required Qualifications
*** Active Physical Therapist (PT) license in the state of Tennessee
* Minimum 1 year of PT experience
* Strong communication and assessment skills
* Ability to work independently with a flexible PRN caseload
* Valid driver’s license and reliable transportation
** Preferred Qualifications
*** Home Health experience
* Experience with EMR documentation systems
* Strong understanding of rehabilitation principles and functional mobility## ##
