At Home Private Duty Caregiver – Avenues Home Care
Join the front line of compassionate care as an At Home Private Duty Caregiver with Avenues Home Care, formerly Sentinel Homecare. Based in Nashville, TN, you will provide dedicated support to elderly, vulnerable, and disabled clients across the local area, including Nashville, Belle Meade, Green Hills, Franklin, Goodlettsville, Hendersonville, Gallatin, Lebanon, Mount Juliet, Murfreesboro, Smyrna, Ashland City, Hermitage, and beyond.
Responsibilities- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, personal hygiene, toileting, transferring, eating, and continence care.
- Support medication administration, housekeeping, laundry, meal preparation, feeding, shopping, transportation to appointments, and companionship.
- Perform other household tasks as outlined in the client’s plan of care.
- Report daily activity to your supervisor via the agency management system and activity logs; use phone, email, agency app, or face‑to‑face communication as needed.
- Practice safety in all activities; lift or push up to 50 pounds; maintain honesty, accountability, and integrity.
- Provide superior care; stay vigilant for care concerns or changes in condition; adhere to dress code and personal appearance standards.
- Be timely in reporting for shifts or making scheduling changes; report suspected abuse or neglect immediately.
- Document any unusual behavior, events, conditions, or people; maintain confidentiality of records and correspondence.
- Attend meetings and in‑services as requested; perform other duties as assigned.

- Fit for duty: valid driver’s license, automobile insurance, and the following minimum requirements:
- 18 Years of Age or Older
- CNA License or alternate experience preferred
- CPR & First Aid Certifications preferred
- Reliable Transportation
Prior experience assisting with ADLs or caring for elderly, disabled, and vulnerable clients.
