The Clinical Pharmacist – PT, PD, CA is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry‑level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
Responsibilities- Determine the suitability of individual patients for home care, collect and organize all patient‑specific information needed to determine suitability and assess patients’ suitability for home care in accordance with Option Care policies.
- Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients, making suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters as appropriate.
- Modify the pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information, competently applying pharmacokinetic principles, formulas, and laboratory values.
- Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques.
- Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, prepare and dispense medications according to Option Care policies and procedures, and adhere to state/federal laws and USP standards.
- Manage the use, maintenance and troubleshooting of medication administration equipment and related equipment, accurately program and maintain all equipment, report medication‑related incidents and errors, and complete QAR documents per Option Care policy.
- Provide concise, timely responses to requests for drug information from health care providers and home care patients, formulate responses based on literature analysis, and attend educational seminars to meet CEU requirements.
- Manage the use of investigational drug products according to established regulations and Option Care policies, utilize pharmacy support personnel effectively, prioritize work, and ensure accuracy of the work of support personnel.
- Comply with all accreditation, legal, regulatory and safety requirements in all activities.

BasicEducation and/or Experience Requirements
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: (a) At least one year of experience in sterile IV admixture procedures; (b) Knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM); (c) Completion of an ASHP accredited Pharmacy Residency Program; (d) Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or (e) Equivalent experience approved by the Vice President (VP) of Pharmacy and/or Senior Vice President (SVP) of Clinical Services.
Qualifications
- Reliable means of transportation enabling home visits and a valid driver’s license and proof of insurance if a personal vehicle is used.
- Experience providing customer service to internal and external customers, including meeting quality standards for services and evaluating customer satisfaction.
- Basic level skill in Microsoft Word (e.g., opening documents, cutting, pasting, aligning text, inserting bullets, using find and replace, spell check).
