This department includes a dedicated team of Case Managers, Social Workers, and Interpreters that ensure that all care is equal, and our patient's dignity is maintained above all else.
** About the Job:
** The RN Acute Care Case Manager evaluates the patient’s health status and the patient/family/significant other’s care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage.

* What You’ll Do:

*** Assesses and identifies patients’ needs during the discharge planning process
* Provide resources and community referrals for continuity of care
* Care coordination with families, providers, and clinical team
* Utilization Reviews
** Who You Are:
*** ADN or BSN required  + BSN preferred but not required
* 3 years of case management-related health care experience.
* Experience with patient documentation in an EMR system preferred

** Days, 1st shift, 32 hours weekly, every other weekend rotation
