About the Job

The RN Embedded Care Coordinator – Oncology plays a vital role in managing care for complex hematology/oncology patients. This position integrates direct nursing care, proactive care planning, and multidisciplinary coordination to support patients across the cancer care continuum. The role is embedded within the oncology practice and focuses on improving outcomes through education, advocacy, and clinical excellence.

What You’ll Do
  • Develop and monitor individualized care coordination plans
  • Collaborate with providers to manage treatment and follow‑up
  • Identify and support patients with co‑morbidities or psychosocial needs
  • Educate patients and caregivers on disease, treatment, medications, and symptom management
  • Perform telephone triage and coordinate interventions with the Oncology Nurse Navigator
  • Monitor labs, test results, and treatment adherence
  • Assist with serious illness and end‑of‑life discussions
  • Document care interventions and outcomes in a timely manner
  • Participate in Tumor Board, Cancer Committee, and quality improvement initiatives
  • Maintain compliance with HIPAA, infection control, and safety protocols
Who You Are
  • A compassionate, patient‑centered RN with a BSN (required)
  • Licensed in New Hampshire with current BLS certification
  • OCN certification (preferred or within 2 years of hire)
  • Chemo/immunotherapy certification (within 1 year of hire)
  • At least 2 years of healthcare experience, preferably in oncology
  • Skilled in critical thinking, communication, and care coordination
  • Familiar with EPIC and Windows‑based software
  • Able to work independently and collaboratively in a fast‑paced environment
