Overview

We are seeking a Primary Care Physician to join our team in a patient-centered medical home model, collaborating with pharmacists and nurse case managers to support high-risk populations. Our facility offers on-site diagnostics (x-ray, ultrasound, 3D tomosynthesis), outpatient procedures, and seamless care transitions with nearby Express Care. Additional services include on-site Ob/Gyn, dermatology, and physical therapy, fostering comprehensive clinical collaboration. The ideal candidate is board-certified or eligible in Internal or Family Medicine
