Monday-Friday Full time TBD

• Competitive salary, 53K-63K negotiable based on relevant experience

• Provides case management services to members with chronic or complex conditions
• Conducts assessment of the member, member’s family/caregiver, hospital staff, physicians and other providers
• Develops and implements a case management plan
• Performs ongoing monitoring of the plan of care to evaluate effectiveness
• Evaluates effectiveness of the care plan and modifies as appropriate to reach optimal outcomes

• MUST be an RN with 2+ years of experience
• 1-2+ years of Case Management experience or Hospital background coordinating care, discharge planning etc.
• Interqual or Milliman experience preferred
• Experience working with Complex/Chronic conditions
• Must have clear driver’s license
