We are seeking a compassionate, dedicated, and highly skilled Registered Nurse (RN) to join our home care team as a Home Care Field Supervisor
. This role is essential in ensuring our clients receive exceptional, safe, and consistent care in the comfort of their homes. As a Field Supervisor, you will provide clinical oversight, support caregivers in the field, and ensure all services meet state regulations and agency standards.

Key ResponsibilitiesConduct in-home supervisory visits with clients to assess care quality, safety, and client satisfaction.
Perform initial assessments, re-assessments, and ongoing evaluations of client conditions and needs.
Develop, update, and implement individualized care plans according to state requirements and agency guidelines.
Train, mentor, and support field staff—including HHAs, CNAs, and other caregivers to ensure competency and compliance.
Provide clinical guidance and act as a resource for caregivers regarding client needs, care techniques, and best practices.
Monitor documentation for accuracy, completeness, and compliance with state regulations.
Maintain timely communication with office staff, clients, family members, and care teams.
Participate in on-call rotation as needed to support the agency and ensure continuity of care.
Identify concerns in the field and take appropriate corrective actions to maintain high standards of service.
Ensure all care is delivered in accordance with company policies, licensing standards, and ethical practices.
Must hold an active, unrestricted Registered Nurse (RN) license.
Minimum 1 year of nursing experience required (home care experience preferred).
Strong understanding of home care regulations, care plans, and assessment procedures.
Excellent communication, leadership, and interpersonal skills.
Ability to work independently, manage time effectively, and handle multiple client needs.

Valid driver’s license and reliable transportation for field visits.
