Position Summary:

We are currently seeking a Full Time Registered Nurse (RN) to join our home health team at a building-specific location in South Naples. The ideal candidate will be responsible for providing high-quality, compassionate care to patients in their homes and ensuring their safety and well-being.

• Conduct initial and ongoing assessments of patients’ health status and develop individualized care plans
• Administer medication, treatments, and other nursing interventions as prescribed by the patient’s physician
• Monitor patients’ conditions and provide necessary education and support to help them manage their health
  
  Document all patient care activities in accordance with company policies and regulations
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients
• Communicate regularly with patients, families, and caregivers to provide updates on the patient’s progress and address any concerns

• Current Florida RN license
• Home Health Experience required
• Minimum of 1 year of experience working as a Registered Nurse, preferably in a home health setting
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently as well as part of a multidisciplinary team
• Reliable transportation and willingness to travel to patient’s homes
